Practice Fusion Partners with Merck on Population Health Management Initiative

Practice Fusion launches Population Health Management

The average American doctor sees 2,000 patients per year. On paper, each patient’s information is locked, and the doctor has no ability to look across their patient population to see who’s in need of critical care.

Today, Practice Fusion has teamed up with Merck to launch a new Population Health Management program for the provider community that gives the ability to monitor the health of their entire patient population in real time. Using a real time dashboard, Practice Fusion’s 112,000 monthly active medical professionals can now actively track the percentage of their adult patients that have received select vaccinations based on guidelines from the Centers for Disease Control and Prevention (CDC).
The system then introduces an alert system to provide clinical decision support at the point of care. On a macro level, this brings a new level of personalized, proactive care to small- and solo-practice doctors, with a significant opportunity to improve patient outcomes and potentially save lives.

Practice Fusion is rolling out the first phase of the program this month, which will cover vaccines recommended for adult patients. The need for this program is dire: more than 40,000 adults die each year in the U.S. from vaccine-preventable diseases (Source: Healthy People 2020,www.HealthyPeople.Gov). The alerts we send tell doctors that one or more of their patients may be due for a critical vaccination.
